Abstract

The most important cause of thunderclap headache (TCH) is subarachnoid hemorrhage (SAH), but this type of headache had other important causes. If you have the following signs and symptoms associated with TCH, you should consider secondary causes: the decreased level of consciousness, focal neurological deficits, and seizures. Some of these secondary causes include SAH, reversible cerebral vasoconstriction syndrome, spontaneous intracranial hypotension, and cerebrovascular events such as arterial dissection, cerebral venous thrombosis, and ischemic stroke. If all the secondary causes of this type of headache are ruled-out, it is called primary TCH. In this chapter, we describe the definition of TCH, its diagnostic algorithm, the necessary diagnostic studies for its various causes, and its treatments. © 2022 Elsevier Inc. All rights reserved.
