Abstract

Iodine deficiency during lactation can have serious consequences, the most important of which include reduced intelligence quotient and impaired physical growth and development in infants and young children. Accordingly, this policy brief has been developed to draw the attention of policymakers and health stakeholders to the need to ensure adequate iodine intake during lactation. Evidence from Iran indicates that although the country has successfully eliminated iodine deficiency in the general population through the universal salt iodization program, lactating mothers remain at risk of inadequate iodine intake. Therefore, a set of key actions is essential to prevent and control iodine deficiency during this critical period. These include strengthening oversight of the production and distribution of standardized iodized salt and preventing the marketing of non-iodized salt; implementing regular monitoring of iodine status in lactating mothers; and expanding laboratory capacity to measure breast milk iodine concentration alongside urinary iodine assessment. In addition, full implementation and supervision of the national iodine supplementation guidelines for lactating mothers, the provision of free or subsidized iodine supplements or iodine-containing multivitamins through health care centers, and public awareness campaigns on the importance of adequate iodine intake during lactation are of high priority. Furthermore, supporting new research on the short-and long-term effects of iodine supplementation during lactation on maternal and child health can provide critical evidence to strengthen policymaking and guide future interventions. © 2026, Research Institute for Endocrine Sciences.
